Are you faced with making life changing decisions for a family member? Our 'relative Support Service' may be able to help.
When a family member becomes unwell, their situation can often reach crises point, leaving relatives to make life changing decisions on their behalf. The Relative Support Service will help families access support to make informed choices about future care for their loved ones.

This service is for: Relatives with an adult family member in a West Sussex hospital

How can this service help me?
Most people who have to make decisions, or support a loved one to make decisions, about future care and housing have limited knowledge of the processes that help them access the support the need. The Relative Support Service simplifies the system, providing information and advice that ensures families are supported during the transition between hospital, residential care or care in the community.

We will:
* Talk to relatives in the hospital
* Take all the details of their circumstances
* Provide a range of factsheets and guidelines relevant to their situation
* Assist carers to make contact with other services that could be of benefit, i.e. Carers Support West Sussex
* Arrange a priority referral to the appropriate Age UK West Sussex or Guild Care support service if additional help is required
* Provide information on appropriate housing, care home options or care agencies and support relatives to make informed choices
* Outline the pathway between hospital and care

Additional support can be provided by a telephone consultation or a face to face appointment.
