Requesting an assessment in Westminster

If you would like information about an assessment either for yourself or someone you know, or a carer's assessment if you or someone you know is a carer, you can contact the relevant team within Westminster as follows:

If you want to start the ball rolling by making a referral to the Westminster Adult Social Care team then you can complete this referral form and email it to above.

EMERGENCY / OUT-OF-HOURS TEAM 

Outside normal office hours this team deals with statutory Mental Health Assessments, other emergencies concerning vulnerable adults and elderly residents, child protection emergencies and other children and family issues.

The team works with people with learning disabilities aged 18 and over and their family carers to identify where support is needed and how this can be provided in line with an individual’s person-centred plan.

Westminster Dementia Service have produced a leaflet called Living well with dementia 

WESTMINSTER OLDER PEOPLE COMMUNITY MENTAL HEALTH TEAM
(supporting people over the age of 65 with a confirmed diagnosis of dementia and more advanced symptoms, and people over the age of 65 with other mental health diagnoses. Initial contact usually comes following a referral from your GP or another health or social care professional)
