OLDER PEOPLE'S MENTAL TEAM (AKA OLDER PEOPLE AND HEALTH AGEING SERVICE)
(supporting people over the age of 65 with a confirmed diagnosis of dementia and more advanced symptoms, and people over the age of 65 with other mental health diagnoses. Initial contact usually comes following a referral from your GP or another health or social care professional)
